Weeks 1–2
Pain
Get the leg pain to back off.
Manual therapy, spinal manipulation and dry needling calm the irritated nerve and the muscles clamping around it. Treatment starts on visit one.
What We Treat
Standing up off the couch like you’re eighty. The burning line down one leg deciding how far you walk today. It’s fixable, and it starts with a cause-first evaluation, not a guess.

Why It Happens
Your back doesn’t hurt for no reason, and it’s almost never one single thing. Usually it’s some mix of four. An irritated disc pressing on a nerve, which is what sends that line of fire down your leg. Spinal joints that got stiff and stopped sharing the load. Deep stabilizing muscles that quietly switched off after your last flare-up. And the daily habits, how you sit, bend and lift, that keep reloading the same spot before it ever heals.
None of that shows up in a glance. It takes a cause-first evaluation to sort out which one is actually driving your pain. You’re the one who handles things, which is exactly why you’ve been managing this with ibuprofen and a foam roller since spring. Find the real cause and the fix gets obvious.
The Honest Truth
Nobody walks in here wishing they had waited longer. The sentence we actually hear is “I wish I’d done this sooner.”
How We Fix It
Every plan has an endpoint. Here’s what the four phases look like for back pain and sciatica.
Weeks 1–2
Pain
Manual therapy, spinal manipulation and dry needling calm the irritated nerve and the muscles clamping around it. Treatment starts on visit one.
Weeks 2–6
Prime
Targeted mobility work returns motion to the stiff segments that have been guarding, and to the hips that stopped helping them.
Weeks 6–10
Perform
Progressive core and hip strengthening builds the stability your spine was missing before any of this started.
Weeks 10+
Prevent
Lifting, bending and twisting get tested under real load before you’re cleared to go all-in. That’s the endpoint, not just another appointment.
I started at Revival Performance six months ago with back pain, and from day one, they've been incredibly proactive. In my first appointment, they didn't just hand me a list of exercises, they walked through my symptoms, showed me how to move safely throughout my day to reduce pain, and provided detailed explanations for every part of the plan.

Questions, Answered
Usually not, and it’s rarely the first step. Studies keep finding disc bulges and wear and tear on the scans of people with zero pain at all, so an image on its own doesn’t tell you what’s driving yours. We start with a hands-on, cause-first evaluation to find what actually reproduces your symptoms. If imaging becomes genuinely necessary, we’ll help you get it. You don’t have to wait on a scan to start feeling better.
Yes, in most cases movement helps more than rest does. Hurt doesn’t always mean harm, and total rest tends to leave a back stiffer and weaker, which sets up the next flare-up. The skill is dosing the right movement for your specific back. That’s exactly what a Doctor of Physical Therapy sorts out at your evaluation, then progresses with you as you improve.
Because the leg is where you feel it, not usually where it comes from. Sciatica is a nerve being irritated somewhere along its route, most often up at the spine or deep in the hip. Chasing the calf with stretches and rollers treats the loudest spot instead of the source. We test the whole route, find where the nerve is actually getting squeezed, and treat that.
You get a plan with an endpoint, and you’ll have it before you leave your evaluation. Every plan runs the same four phases: Pain, Prime, Perform, Prevent. How long each one takes depends on how long you’ve had this and how your back responds. You’ll always know which phase you’re in and what’s left, instead of an open-ended string of appointments.
