Revival Performance Physical Therapy has two San Antonio locations.

Westwood Loop: 11121 Westwood Loop, Suite 104, San Antonio, TX 78253. Phone (210) 972-9130.

Both locations keep the same hours, fax (210) 866-0201, and email, info@revivalperformancept.com.

At Heritage Blvd, Suite 160 is the one you want. Give yourself a couple of extra minutes on your first visit to park and find the door. If anything is confusing, call from the lot and someone will point you in.

What happens when you call

You tell us what hurts and how long it has been going on. That is the whole ask. No script, no gatekeeping.

We will talk through whether we are the right fit, answer the cost question honestly, and offer to run a benefits check so you know the number before your first visit.

Then we find you a time. If you would rather not talk yet, request an appointment and we will call you.
