What this policy covers

This is the privacy policy for the Revival Performance Physical Therapy website. It explains what this site collects, how we use it, and who else is involved.

It is a website privacy policy. It is not our HIPAA Notice of Privacy Practices. That is a separate document about your protected health information as a patient, and you receive it at your first visit. If you would like a copy before then, ask us and we will provide one.

What we collect

Two kinds of information.

First, what you type in. If you fill out a form on this site, we receive what you entered. That may include your name, phone number, email address, and whatever you tell us about your situation.

Second, what your browser reports automatically. Analytics tools record things like which pages were viewed, roughly where in the world the visit came from, what device and browser were used, and which site referred you.

  • Contact and appointment request forms: name, contact details, and the message you write
  • Benefits check requests: the insurance information you choose to provide
  • Analytics: page views, session data, approximate location, device and browser, referring source
  • Cookies and similar technologies used by analytics and by the scheduling tools we use

Health information in forms

Some of our forms invite you to describe what hurts. That means a form submission may contain health information.

We treat those submissions as confidential clinical inquiries, not as marketing data. They go to our staff so we can respond to you and, if you become a patient, so we can prepare for your visit.

A word of caution that applies to any website. Email and web forms are not fully secure channels. Please share only what you need to in order to start a conversation. Anything sensitive is better said on the phone at (210) 750-9004 or in person.

How we use it

  • To answer your question or return your call
  • To schedule, confirm or change an appointment
  • To run a benefits check when you ask for one
  • To send you information you asked for
  • To understand which parts of this site are useful and fix the parts that are not

We do not sell your information. We do not rent it. We do not trade it.

Who else is involved

Running a website means using other companies. These are the ones that touch data from this site.

  • Netlify, which hosts this website and processes form submissions made through it
  • Google Analytics, which measures site traffic and usage
  • Our patient scheduling and communication portal, which handles appointments, messaging and home exercise programs for current patients
  • Embedded third-party content, such as a Google review widget and a map, which may set its own cookies

Each of these providers has its own privacy policy governing what it does with data it handles.

Text messages

If you give us your mobile number and consent to text messages, we may text you about appointments and your care. Consent is not a condition of receiving treatment. You can opt out at any time by replying STOP or by telling us. Message and data rates from your carrier may apply.

How long we keep it

Website enquiries are kept as long as we need them to respond to you and to keep an accurate record of that contact.

Patient records are a separate matter. Those are clinical records, and they are retained according to Texas law and professional requirements, not according to this policy.

Your choices

  • Ask us what information from this website we hold about you
  • Ask us to correct it if it is wrong
  • Ask us to delete a website enquiry or form submission
  • Ask us to stop contacting you
  • Block or clear cookies in your browser, and use your browser or platform controls to limit analytics

One limit worth being straight about: a deletion request for website data does not delete a clinical record. Health records are governed by separate rules and we cannot remove them on request.

Children

This website is not directed at children, and we do not knowingly collect information from children through it. We do treat minors in the clinic, and a parent or guardian handles that paperwork with us directly.

Where we link out, that other website has its own policy and we are not responsible for it.

Changes to this policy

If we change how this site handles information, we will update this page. Using the site after a change means the updated policy applies.
